Non unique names in input are not handled correctly

When exporting work items, the customized fields do not have garanteed unique names, e.g. we have customized the field "Found in".
Now when exporting work items, the field names are taken over, from the labels, but the labels are not garanteed unique.
Consequently, there are two columns with the same name "Found in" in the CSV table file.

When selecting such a file for import, there is not error message provided after analysing the file.
The error occurs late when trying to save the mapping.
However, in this case, there is no more chance to save the work of configure mapping of the fields (which takes several minutes),
so the user's work is lost.

Expected solution: When loading the input file (i.e. reading the input column names), duplicate field names shall be detected and an adequate error message shall be reported.
