Allow multiple Failure Reports for a single work order by making it easier to add a failure report to an activity or task

You may need to enter multiple Failure Reports for a single work order. You may want to make a failure report against a component of an asset without changing the asset referenced on the work order. The root cause of a failure may occur on an asset which was not referenced on the work order. You may not wish to change the work order's asset, it could change service address, GL Account, etc, with undesired consequences. Work Edit settings may not let you change the asset.

There is a solution to these issues in the current Maximo system but it is not obvious. You can create a task at COMP status (not at CLOSED), but you need to navigate to the Activities and Tasks application, enter a different asset and then enter the failure report. For many clients Activities and Tasks is not enabled. 

The solution is to provide conditional UI on the Work Order Tracking application, Failure Reporting tab - Multiple Failures? which when set, displays the Tasks table window allowing a task to be created, an asset entered, and a failure report raised against the task and asset. It should be possible for a reviewer to see both the failure report made against the original work order as well as the failure reports made against the tasks and assets that were the actual cause of failure.

Ideally we should also record the person and date/time when the Cause Code is entered.
